Which ban system do you use, and why do you use the one you use over another?

I've seen systems such as:
  • 3 strikes and then ban
  • Ban without any other strikes
Which one are you using on your forum?

P.s. Obviously, some offenses will warrant a first-time ban, but for the minor offenses, are you giving a few warnings before banning or just handing out bans and not dealing with it anymore?
